Machine safety – Bypassing the safety device

In this animation we will be looking at a fatal occupational accident based on TOT 2/19 report. The report presents the view of the investigation team on the events leading to the accident and risk factors and recommendations for the prevention of similar accidents.

The objective of the TOT-investigation (TOT stands for investigation of accidents at work) and the case report is to improve the prevention of occupational accidents. The purpose of the report is not to apportion blame or liability of the parties. Unfortunately TOT-reports are only available in Finnish.
